I just switched my hosting from Netfirms to ASO.

I have the following form email (which obviously doesn't work anymore):

Can someone please explain like I'm five: what's the easiest way to get this form operational?

There is a script available through cpanel called cgiemail that should do what you want. There are numerous other e-mail form scripts about the net as well.
Obsolescence is just a lack of imagination.
